About the PositionWe are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment provider. In this role, you will conduct comprehensive health assessments, review medical histories, and provide personalized care plans to help our members achieve better health outcomes.
As a Home Risk Assessment provider, you will work collaboratively with our team to identify and address health risks, provide education and support to our members, and facilitate connections to community resources.
Responsibilities- Conduct comprehensive health assessments, including medical history reviews and physical examinations
- Develop and implement personalized care plans to address health risks and promote healthy behaviors
- Provide education and support to our members on healthy lifestyle choices, disease management, and medication adherence
- Collaborate with our team to identify and address health risks, and facilitate connections to community resources
- Document all interactions and assessments in our electronic health record system
- Master's degree in Nursing or related field
- Current RN and NP licensure in state of practice
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health record systems
- Excellent verbal and written communication skills
This position requires frequent travel to meet patients in their homes, skilled facilities, and through telehealth visits. A valid state driver's license and reliable transportation are required.
