Job Summary

The Peer Support Specialist is a core member of the Collaborative Care team, working closely with patients, primary care providers, licensed social workers, and other behavioral health providers. This role provides peer support services to individuals with mental illness and/or substance use disorders, focusing on enhancing recovery and supporting treatment goals.

Key Responsibilities
  • Provide peer support services to individuals and groups, using a harm reduction approach to addiction care.
  • Work with patients to optimize care management through care planning, transitions support, and coordination.
  • Use a world-class technology platform to engage patients in treatment and provide individualized brief interventions, guidance, and coaching.
  • Connect patients with community resources and support their recovery goals.
Requirements
  • High School Diploma or GED equivalent, with a preference for a Bachelor's degree.
  • Past or present consumer of mental health or substance use services with a history of managing one's own recovery.
  • Awareness of the importance of recovery in living with a mental illness or substance use disorder.
  • Familiarity with or use of medications for addiction treatment is preferred.
  • Familiarity and experience in working with individuals with substance use disorders.
Preferred Qualifications
  • 1-5 years of experience as a peer support specialist.
  • Additional certification or training in mental health and/or SUD/addiction treatment services.
  • Familiarity and experience of working within a Collaborative Care Model.
  • Familiarity and experience of working within a Chronic Care Management program.

Selection Process

The selection process consists of three stages: a 30-minute virtual interview, a 30-minute virtual assessment interview, and a 45-minute virtual interview with Clinical and Operations Leaders.
