The RN Case Manager Hospice Field position is available at Corewell Health South - Lakeland Hospitals. This full-time role requires assessing, planning, implementing, coordinating, monitoring, and evaluating the strategies and services required to meet the patients' end-of-life care needs.
The ideal candidate will possess a strong understanding of symptom management and end-of-life care, as well as excellent communication and interpersonal skills. A minimum of 1 year of relevant clinical experience is required, and hospice experience is preferred.
This rewarding role offers a comprehensive benefits package, including on-demand pay, discounts directory, and traditional and Roth retirement options. The estimated salary for this position is $75,000 per year, based on the location and requirements of the job.
The primary location for this position is SITE - Hospice & Palliative Care Royalton - 4025 Health Park Lane - St Joseph. Team members are expected to work a standard 40-hour week, Monday through Friday.
