Hospice RN Job Description:
We are seeking a dedicated and compassionate RN to join our Hospice team at St. Charles Health System. As a Hospice RN, you will be responsible for providing high-quality patient care, developing and implementing individualized care plans, and collaborating with the interdisciplinary team to ensure seamless care delivery.

About the Role:
This full-time position involves working with patients and their families to assess their physical, emotional, and spiritual needs, developing and implementing care plans that address these needs, and providing education and support to patients and families. You will also be responsible for maintaining accurate and detailed patient records, communicating effectively with patients, families, and caregivers, and addressing any concerns or issues that arise.

Key Responsibilities:
  1. Assess patients' physical, emotional, and spiritual needs, developing and implementing individualized care plans
  2. Collaborate with the interdisciplinary team to ensure seamless care delivery and address any concerns or issues
  3. Maintain accurate and detailed patient records, documenting all aspects of care provided
  4. Communicate effectively with patients, families, and caregivers, addressing any questions or concerns

Requirements:
To be successful in this role, you must possess a current Oregon RN license, a valid driver's license, and a strong understanding of hospice care principles and practices. Previous experience in hospice care is preferred, but not required. This position requires excellent communication and interpersonal skills, with the ability to work independently and as part of a team.

Estimated Salary Range:
$65,000 - $85,000 per year, depending on experience and qualifications.
