Key Responsibilities:
  • Assume full nursing responsibility for delivering personalized care plans to each hospice patient.
  • Conduct thorough initial evaluations to determine eligibility for hospice services within 48 hours of referral.
  • Identify and address patients' physical, psychosocial, and environmental needs through regular reassessments (at least every 15 days).
  • Collaborate with patients, families, attending physicians, and other healthcare professionals to provide comprehensive care.
  • Maintain accurate clinical notes and document patient progress.

$83,000 - $101,000 per year, depending on experience
