Job Description:

Community Care Worker I - Are you a highly motivated, compassionate and dedicated individual looking for a rewarding career determining eligibility for the most vulnerable citizens of Texas in need of in-home care, home delivered meals, emergency response services, and other social services?

Key Responsibilities:

  • Determines needs and eligibility for Community Care for the Aged and Disabled (CCAD) services.
  • Conducts home visits, interviews individuals who are older or have a disability, documents information, determines need for services and develops service plans.
  • Verifies data, explains program benefits and requirements, refers individuals for appropriate services, and assures authorizations are registered in automated systems.
  • Authorizes and monitors CCAD services to determine if services are meeting the individual's needs.

Requirements:

  • High School Diploma or GED equivalent is required.
  • Prefer experience indicating the individual possesses the knowledge, skills and abilities necessary for job performance.
  • Ability to effectively communicate orally and in writing.
  • Ability to set priorities, establish timeframes, and meet deadlines.
  • Ability to establish and maintain effective relationships with individuals receiving services, co-workers, contract agency staff, and staff from other federal or state agencies.
