- Collaborate with healthcare teams to develop and implement patient care plans.
- Conduct medication use evaluations and develop formulary recommendations.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Doctor of Pharmacy degree from an accredited university.
- Completion of a clinical pharmacy residency or fellowship.
- Current, valid, unrestricted license to practice pharmacy in any state.
- At least 2 years of experience in clinical pharmacy practice.
- BLS certification.
