The Fairbanks Resource Agency Inc is seeking a highly skilled and compassionate Supported Living Specialist to join our team. As a Supported Living Specialist, you will play a vital role in providing and coordinating supports that enable our participants to live as independently as possible.
Key Responsibilities- Assist in the planning, implementation, and documentation of Plans of Care (POCs) to ensure the well-being, safety, and quality of life of program participants.
- Provide guidance and training that facilitates independence and recognizes and respects the rights of participants.
- Collaborate with participants, families, and other stakeholders to develop and implement personalized support plans.
- Conduct regular home visits and assessments to monitor participant progress and adjust support plans as needed.
- Develop and maintain relationships with community resources and service providers to ensure participants have access to necessary services.
- Participate in team meetings and contribute to the development of policies and procedures that promote participant-centered care.
- Bachelor's degree in a related field (e.g., social work, psychology, or a related field).
- Minimum of 2 years of experience in a related field, preferably in a supportive living or care coordination role.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork.
- Valid driver's license and reliable transportation.
- Ability to pass a thorough background check and maintain confidentiality and professionalism at all times.
- Experience working with individuals with disabilities or chronic illnesses.
- Knowledge of community resources and services, including healthcare, education, and social services.
- Ability to speak multiple languages, with a preference for fluency in English and Spanish.
- Experience with electronic medical records (EMRs) and other software applications.
