Job Title: Homecare Nurse and Caregiver Supervisor

We are seeking a qualified Licensed Practical/Vocational Nurse (LPN/LVN) to fill the dual role of Homecare Nurse and Caregiver Supervisor under RN supervision.

Responsibilities:
  • Conduct comprehensive assessments of clients' medical needs, develop individualized care plans, and monitor their progress.
  • Serve as a primary point of contact for clients, families, and caregivers, coordinating all aspects of care delivery and ensuring continuity of services.
  • Provide hands-on training and supervision to home care aides, including instruction on personal care tasks and safety protocols.
  • Conduct in-home visits to assess caregiver competency, provide ongoing support and education, and ensure adherence to care plans.
  • Monitor medication administration and medication management.
  • Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to optimize client outcomes and promote holistic care.
  • Maintain accurate and thorough documentation of client assessments, care plans, and interventions in compliance with regulatory standards and organizational policies.
Qualifications:
  • Current licensure as a Licensed Practical/Vocational Nurse (LPN/LVN) in the state of Washington.
  • Minimum of two years preferred, of clinical experience, with a preference for candidates with prior experience in homecare or case management.
  • Strong critical thinking skills, with the ability to develop and implement individualized care plans.
  • Excellent communication and interpersonal abilities, with a compassionate and client-centered approach to care.
  • Experience in training and mentoring caregivers preferred, with a commitment to promoting excellence in caregiving practices.
  • Proficiency in computer skills, including electronic health record documentation and Microsoft Office.
