Key Responsibilities
  • Provide patient-centered care to individuals calling or presenting for care through comprehensive assessments, development of personalized plans, and execution of evidence-based interventions.
  • Collaborate with multidisciplinary teams to ensure seamless transitions of care and optimize patient outcomes.
  • Maintain accurate and up-to-date records, ensuring compliance with regulatory requirements and organizational standards.
Requirements

To be successful in this role, you will need:

  • A graduate degree in nursing from an accredited institution; a Bachelor's degree in Nursing is preferred.
  • A current state license as a Registered Professional Nurse; current BLS certification is required, or must be obtained within 30 days of hire date.
  • At least one year of experience as a Registered Nurse; additional experience is highly valued.
