We are currently seeking a Registered Nurse to join our Ambulatory Care team. As a Care Coordinator RN, you will be responsible for providing high-quality care to patients and families through assessment, planning, implementation, and evaluation.

Key Responsibilities:

  • Provide comprehensive nursing care to patients and families, including assessment, planning, implementation, and evaluation
  • Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans
  • Coordinate patient care activities, including scheduling appointments and procedures
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination

Requirements:

  • Graduate of an accredited School of Nursing required; BSN preferred
  • Current applicable state(s) license as a Registered Professional Nurse required
  • Current BLS certification required or must obtain within 30 days of start date
  • One year of RN experience preferred

Salary: $65,000 - $85,000 per year, depending on experience
