Overview: We are seeking a dedicated Psychiatric Rehabilitation Services Coordinator to address the social service requirements of adults in a long-term care environment. This role is essential in fostering a supportive atmosphere for residents.
Key Responsibilities:
- Establish and maintain a stable therapeutic relationship with residents.
- Guide residents in acclimating to the facility.
- Assist residents in comprehending their treatment plans and daily schedules, including group activities.
- Facilitate and encourage active involvement in treatment plan reviews.
- Oversee the delivery of psychiatric rehabilitation service programs.
- Monitor residents' self-care efforts and adherence to treatment plans.
- Complete necessary documentation upon admission and during quarterly and annual reviews.
- Regularly document residents' progress, including incidental notes and monthly evaluations.
- Engage with community organizations for resident referrals.
- Maintain an understanding of residents' psychotropic medications and their intended effects, supporting behavioral programming to enhance functional levels.
- Conduct group and individual counseling sessions as assigned.
- Familiarize oneself with facility policies regarding behavioral emergencies and undergo training in crisis management.
- Participate in educational training sessions and in-service programs provided by the facility.
- Perform additional duties as required.
Qualifications:
- A degree in Social Work or a related field is preferred (BA/BS, MA/MS or higher).
- Availability for flexible hours, including weekend rotations.
- Ability to work effectively within a long-term care setting.
- Exhibit sound judgment under pressure, emotional resilience, and adaptability to changing situations.
Compensation: We offer a competitive hourly wage along with a comprehensive benefits package, including pro-rated benefits based on hours worked.
