Community Based Rehabilitation Specialist

We are seeking a highly skilled and compassionate Mental Health Therapist to join our team at South Central Human Relations Center. As a Community Based Rehabilitation Specialist, you will provide community-based rehabilitative services to children and adolescents with mental illness and their families.
Key Responsibilities- Provide individual, family, and/or group skills training in the client's home, community, or agency setting.
- Develop and implement individualized treatment plans in collaboration with clients, families, and mental health professionals.
- Facilitate or co-facilitate group skills training in an agency setting.
- Maintain accurate records of all billable services and telephone contact with clients and their families.
- Coordinate treatment planning meetings with clients, families, team members, and natural supports as appropriate.
- Receive clinical supervision from a qualified mental health professional.
- Meet the Mental Health Practitioner requirements as per MN State Statute 245I.
- Hold a master's degree or other graduate-level degree in one of the behavioral sciences or related fields, or a bachelor's degree in one of the behavioral sciences or related fields with 2000 hours of supervised experience in the delivery of mental health services.
- Have 4000 hours of supervised experience in the delivery of mental health services to people with mental illness.
- Meet the clinical trainee requirements per MN State Statute 245I.04.06.
- Have a valid driver's license, auto insurance, and a reliable vehicle.
- Be proficient in the use of computer programs, including Microsoft Word, Excel, Outlook, and Electronic Medical Record (EMR).
