Key Responsibilities:

  • Schedule and attend medical appointments for individuals with IDD
  • Provide transportation to healthcare providers
  • Document and communicate with nursing and residential departments
  • Advocate for individuals at appointments
  • Track and ensure appointments are kept or rescheduled

Requirements:

  • High School Diploma or GED
  • NYS driver's license that meets Insurance Agency requirements
  • Pass all NYS and OPWDD background clearances
  • Must be med certified or become certified

What We Offer:

  • Competitive pay rate of $17.00-21.50
