Job Title: Occupational Therapist - PRN

We are seeking a highly skilled and compassionate Occupational Therapist to join our team at Vierra Communities. As a PRN Occupational Therapist, you will play a vital role in providing exceptional care to our residents and patients.

Job Summary:

The Occupational Therapist - PRN will be responsible for evaluating and treating patients to optimize their physical abilities and skills. You will work closely with our interdisciplinary team to develop and implement individualized treatment plans that promote independence and quality of life.

Responsibilities:
  • Evaluate patients to identify their physical and functional needs
  • Develop and implement individualized treatment plans to promote independence and quality of life
  • Conduct screenings of patients at regular intervals to determine need for intervention/treatment
  • Establish individual patient schedules in conjunction with other team members
  • Develop treatment plans by identifying a problem list, including long and short-term goals and methods to achieve identified goals
  • Coordinates treatment plan with patient, family and other team members
  • Implements and conducts treatment as outlined in the treatment plan
  • Monitors patient's response to intervention and modifies treatment as indicated to attain goals in accordance with physician's orders
  • Develops appropriate home or community programming to maintain and enhance the performance of the patient in his/her own environment
  • Documents results of patient's assessment, treatment, follow-up and termination of services
  • Adapts treatment to meet the individual needs of the patient
  • Provides/recommends/fabricates adaptive devices, orthotics or prosthetics. Trains patient, family and appropriate staff in the use or application of the above
  • Participates in discharge planning
  • Refers patients for treatment to Certified Occupational Therapy Assistants (COTA) by completing evaluation and initial plan of treatment and communicating necessary and sufficient information
  • Supervises and directs COTA in the implementation of the treatment plan
  • Supervises Rehabilitation Aides
  • Reviews the quality and appropriateness of the total services delivered and of individual occupational therapy programs for effectiveness and efficiency, using predetermined criteria
  • Participates in patient, family and staff education
  • Confers formally and informally with other team members in coordinating the total rehabilitation program of the patient
  • Adheres to Facility policies and procedures regarding documentation of occupational therapy services
  • Attend in-service training and facility meetings as required
  • Provides in-service education to the members of the patient care team and to members of the community
  • Assumes responsibility for ongoing continuing education and professional development
  • Provides treatment in accordance with established standards of practice, department procedures and efficiency standards
  • Maintains a safe and clean work area and adheres to facility/corporate safety standards
  • Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional
  • Consistently demonstrates sound judgment in the evaluation, planning, implementation and follow-up of the patient therapy programs
  • Maintains positive relationships and rapport with coworkers, patients, family members and facility personnel
  • Reports to work on time and coordinates schedule to achieve maximum efficiency during assigned shift. Ability to work varying hours, including weekends and holidays
  • Assists with and participates in Continuous Quality Improvement Program as required
  • Adheres to established confidentiality standards as required by Facility concerning community and patient information
Requirements:
  • License to practice occupational therapy within the state
  • Bachelor of Science in Occupational Therapy
  • Successful completion of the National Certification Examination for Registered Occupational Therapists
  • Experience with long-term care and/or geriatric care is preferred
  • Must pass a criminal background check
  • Possess strong communication and interpersonal skills
  • Must have the patience, empathy, and compassion needed to care for elderly patients
  • Ability to work cooperatively with other healthcare professionals
  • Must be able to perform the physical requirements of the position; lift, push, pull, carry heavy objects
Additional Requirements:
  • Must be able to exert 20 to 50 pounds of force occasionally (less than 1/3 of the time), and/or 10 to 20 pounds of force frequently (1/2 of the time), and/or up to 10 pounds of force constantly (2/3 or more of the time) to move objects, equipment and/or patients
  • While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, kneel, bend or crouch; use hands to manipulate tools, equipment, or controls; reach with hands and arms, balance, lift and perform medium to maximum transfer assists with patients
  • The employee is required to have visual and hearing acuity sufficient enough to assess patient safety and ability
