MDS Coordinator Position

We are seeking a skilled MDS Coordinator to support our team at Lubbock Health Care Center. The ideal candidate will have a strong background in geriatric nursing and be proficient in software related to comprehensive assessments and care planning.

Key Responsibilities:

  • Schedule residents for assessments and ensure timely completion of MDS and care plan documentation.
  • Monitor and maintain accurate and up-to-date documentation for all residents.
  • Coordinate and attend regular Medicaid meetings to discuss current residents and pending residents.
  • Perform care plan updates as needed and provide DON and Administrator with Quality Indicators Report.
  • Remain current with industry changes through attendance at workshops and conferences.

Requirements:

  • Licensed Nurse in good standing.
  • Geriatric nursing background.
  • Excellent professional skills, including assessment, hands-on care, and communication.
  • Working knowledge of nursing home regulations.
  • Ability to work independently and as a team member.
