Position Overview


The Clinical Pharmacist plays a vital role in delivering comprehensive clinical and operational pharmacy services aimed at ensuring safe and effective medication therapy across diverse healthcare environments.

This professional evaluates patient-specific information to determine medication requirements, confirms medication orders, and supervises the preparation, dispensing, distribution, and monitoring of all pharmaceuticals.

Key responsibilities include conducting drug therapy consultations, educating patients, families, and staff, facilitating care transitions, and mentoring pharmacy students and residents.


Core Responsibilities:

Clinical Duties:


Engages in direct patient care and collaborates with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to recommend optimal medication use.

Involves in targeted clinical initiatives as defined by the department, such as pharmacokinetic consultations and renal dose adjustments.


Demonstrates expertise in recommending suitable therapeutic interventions for patients across all age groups, including neonates, adults, pediatrics, adolescents, and the elderly.


Monitors designated patient areas for clinical outcomes and assesses pharmacotherapeutic regimens for efficacy and potential adverse drug reactions, suggesting therapy modifications as necessary.


Handles all clinical inquiries from assigned units during the designated shift and documents clinical activities and interventions consistently.


Implements effective cost-saving strategies within the patient care unit operations.


Provides drug information and educational resources to pharmacy staff and healthcare professionals.


Conducts medication reconciliation and gathers medication histories for patients in assigned care areas.


Offers counseling and education to patients and their families.


Serves as the primary mentor for pharmacy residents and students, and assists with departmental orientation for new hires.


Responsible for cross-coverage in other patient care areas as required to ensure adequate clinical support.


Accepts special projects assigned by management to enhance departmental efficiency.



Medication Management:

Adheres to departmental and institutional policies regarding medication dispensing.


Verifies medication orders and dispenses medications accurately, safely, and promptly.


Complies with all relevant departmental, state, and federal regulations concerning controlled substances.


Ensures proper storage of all pharmacy products and oversees the fulfillment of stock requisitions.


Maintains established protocols for quality assurance, security of controlled substances, and hazardous waste disposal.


Independently resolves medication order issues in a timely manner.


Processes and validates medication orders using current information system technology to maintain accurate patient profiles.


Supervises pharmacy technicians to ensure accuracy and safety in medication dispensing.


Reviews all products requiring pharmacist approval prior to dispensing.


Maintains proficiency in hospital computer systems and pharmacy automation technologies.


Conducts 'Unit Inspections' as assigned to ensure proper medication storage and handling throughout the facility.


Collaborates with other departments to integrate services and provide comprehensive care. Participation in improvement initiatives is expected.


Engages in departmental quality improvement activities, including data collection for drug use evaluations and reporting medication incidents and adverse events.


Meets performance goals set by management related to key performance indicators in designated patient care areas, such as reducing length of stay and medication delivery turnaround times.


Participates in meetings, staff development programs, and provides presentations on pharmacotherapy topics as requested by management.


Staffing Requirements:
Must cover a minimum of one shift per pay period and one weekend each month.

Qualifications:
Current pharmacist license in good standing.

Doctor of Pharmacy degree preferred.

Completion of a post-graduate pharmacy residency in hospital pharmacy practice is desirable.

Additional training or certification (such as board certification in a specific practice area) is also advantageous.

A minimum of two years of experience as a pharmacist in a clinical setting is required. Completion of a post-graduate year one pharmacy residency program is equivalent to three years of work experience.
