You will play a crucial role in coordinating participant transportation and external appointments. Collaborating closely with contracted transportation providers, external medical professionals, and the PACE clinic, you will ensure that all participant needs are met effectively. This is a full-time, on-site role, with a typical schedule of working Monday through Friday, 8:00 AM to 4:30 PM. Join us in making a difference


Who is PACE of Southwest Michigan?

PACE (Program for All-Inclusive Care for the Elderly) of Southwest Michigan is a 501(c)(3) organization that seeks to uniquely bridge the health and social supports required to help preserve the independence and dignity of our participants. We provide the necessary medical treatment, support, and social interaction to enable our participants to remain in their own homes for as long as possible. The care team consists of physicians, nurses, pharmacists, social workers, therapists, and transportation staff who collaborate daily to ensure comprehensive care.

PACE was born via a partnership formed by 3 local non-profit organizations; Spectrum Health Lakeland (now Corewell), Region IV Area Agency on Aging, and Caring Circle. Since PACE's inception, the organization has expanded its team to over 90 professionals and currently serves over 250 participants and their families within Berrien, Cass, and Van Buren Counties. The unique PACE model of care is centered around the belief that it is better for the well-being of seniors with chronic care needs to be served in their homes and their community whenever possible.

Requirements:

Education: Associate's degree required. Bachelor's degree preferred
Experience: Two (2) years of supervisory experience required. A minimum of two (2) years of dispatch and/or scheduling experience required.

Number/Type of Employees Supervised: Scheduling and Transportation Coordinators

Licensure, Registry, or Certification Required: Valid MI Driver's License and safe driving record

Special Training: Only acts within scope of practice. Meet a standardized set of orientation and/or individual competencies for the specific position description established by PACE of Southwest Michigan and approved by CMS before working independently.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Specific Requirements and Considerations: May be subject to exposure and handling of infectious waste, diseases, conditions, or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or Hepatitis B.

Key Responsibilities

· Supervise, lead, and develop the transportation and scheduling staff. Engage in performance discussions and foster an environment conducive to growth and discovery. Manage team scheduling, approve timecards, and complete other personnel tasks.

· Implement and maintain compliance with PACE scheduling and transportation standards.

· Manage the customer service process as the liaison between internal staff, contracted providers, and the community.

· Serve as a liaison between transportation and clinic staff regarding appointments and communication with family members and caregivers.

· Proactively identify and resolve issues related to scheduling and transportation.

· Analyze trends and suggest improvements to the PACE scheduling and transportation processes.

· Review and verify all bills for services provided by outside contractors.

· Ensure all attendance and transportation records are maintained in accordance with policy.

· Track and maintain all transportation and scheduling metrics.

· Review incident reports for any transportation-related issues or concerns, following up with the contracted provider and any other necessary parties.

· Document and follow up on driver reports or changes in participants' conditions.

· Actively participate in the Quality Improvement Process (QIP).

· Serve as a member of the Interdisciplinary Team (IDT) and report on relevant scheduling items, such as appointment schedules.

· Be proficient and knowledgeable about the transportation scheduling system and reporting.

· Demonstrate problem-solving skills and flexibility in working with the PACE team to coordinate urgent scheduling and transportation needs.

· Abides by all established PACE of Southwest Michigan policies, rules, and regulations, as well as federal and state regulations that govern the operations of the PACE organization.
