Job Type: Full Time (40 hours per week), Non-Exempt

Work Schedule: Mondays through Fridays 8:30 am to 5 pm

Location: 2637 N 5th St, Philadelphia, PA 19133


Some duties and responsibilities include:

  • Initiate Home assessment visit to referred kinship/foster parent's home, with/without accompaniment of CUA case manager, to determine case acceptance.
  • Conduct initial home visits to assess the physical conditions of the property and assess parent's capability to become a resource parent.
  • Obtain all initial documentation necessary to proceed with certification for prospective Kinship and Resource parents.
  • Screen and pre-approve prospective Kinship and Resource parents, once all documentation has been submitted and deemed acceptable.
  • Bring kinship and resource parent's files to full pre-certification, including a home Study report that describes all documentation found on file.
  • Maintain all documentation for Kinship and Resource parents in order to meet State, County, CUA IOC system transformation model, and APM's best practice model.
  • Ensure that any support needed by the Kinship/Resource parent is in place, such as, nursing support, bilingual services, emergency contact information, other.
  • Ensure that substitute care-givers clearances are in place and up-today information and childcare services.
  • Create a Child's folder with information receive/requested to CUA from Appendix N standards.
  • Respond to on-call responsibilities without incurring difficulties. Exercise prudent and all efforts to assist kinship/resource parent achieve and maintain full certification. Report to Supervisor/Director any incidents that jeopardize the health and safety of the children in placement.
  • Participate in Court hearings, as and if, required, with, and never in lieu of the case manager.
  • Conduct home assessment visits every 3 months, to assess physical structure of the home, Kinship/Resource parent's capability to care for children, psycho-social home condition.
  • Follow up with kinship/resource parent's document completion for License renewal to continue caring for children in placement.

Requirements:
  • EDUCATION: Bachelor's degree in Social Work, Psychology or related field
  • EXPERIENCE: One year providing services to children, youth or families in a behavioral health setting
  • CLEARANCES: Pennsylvania Child Abuse Clearances, FBI, Pennsylvania Criminal Record Clearance, Medical examination and TB test results
  • Bilingual in Spanish/English
  • Preferably Knowledge of PA Chapter 3700
