Job Summary:

We are seeking a dedicated and knowledgeable Dietician to join our team. This role involves working with all dietary requests and consults, with a special focus on serving CalAIM members. The Dietician will provide expert nutritional guidance, develop individualized meal plans, and collaborate with healthcare providers to enhance patient health outcomes.

Essential Functions:

  • Conduct comprehensive nutritional assessments for patients in the CalAIM program.
  • Develop, implement, and monitor individualized nutrition plans tailored to the needs and medical conditions of patients.
  • Provide nutritional counseling and education to patients and their families, focusing on promoting healthy eating habits and managing medical conditions through diet.
  • Review and submit all medically tailored meal referrals across CalAIM.
  • Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient care plans.
  • Monitor and evaluate patient progress, adjusting nutrition plans as necessary to achieve desired outcomes.
  • Keep abreast of the latest research and developments in the field of nutrition, ensuring that all advice and plans are based on current evidence-based practices.
  • Document all patient interactions, assessments, and plans in the appropriate medical records.
  • Participate in the development and implementation of nutrition-related policies and procedures.
  • Conduct group educational sessions and workshops on nutrition and healthy living.
  • Assist in the planning and execution of community outreach programs related to nutrition and wellness.
  • Other duties as assigned
Position Requirements:
  • Bachelor's degree in Nutrition, Dietetics, or a related field.
  • Registered Dietician (RD) or Registered Dietician Nutritionist (RDN) credential from the Commission on Dietetic Registration (CDR).
  • Licensed to practice as a Dietician in California.
  • Bilingual in English and Spanish.
  • Hybrid position. Must be able to work remotely from home and attend staff meetings at Titanium's Garden Grove office. Must be able to travel locally on a quarterly basis to teach classes to Titanium members.
  • Minimum of 2 years of experience in clinical nutrition, preferably in a healthcare setting.
  • Experience working with CalAIM or similar programs is highly desirable.
  • Strong knowledge of medical nutrition therapy for a variety of conditions.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • Proficient in the use of electronic health records and nutrition software.
