Position Summary:

We are looking for a committed and skilled Dietician to be part of our healthcare initiative. This position focuses on addressing dietary needs and consultations, particularly for CalAIM members. The Dietician will offer professional nutritional advice, create personalized meal plans, and work collaboratively with healthcare teams to improve patient health results.

Key Responsibilities:

  • Perform thorough nutritional evaluations for patients enrolled in the CalAIM program.
  • Design, execute, and assess tailored nutrition strategies based on individual health requirements.
  • Deliver nutritional education and counseling to patients and their families, emphasizing healthy dietary practices and management of health conditions through nutrition.
  • Process and oversee all medically tailored meal requests within the CalAIM framework.
  • Collaborate with medical professionals to formulate and implement comprehensive patient care strategies.
  • Track and assess patient progress, modifying nutrition plans as needed to achieve optimal health outcomes.
  • Stay updated on the latest advancements in nutrition science, ensuring that all recommendations are grounded in current evidence-based practices.
  • Maintain accurate documentation of patient interactions, assessments, and nutrition plans in medical records.
  • Contribute to the development and execution of nutrition-related policies and procedures.
  • Facilitate group educational workshops on nutrition and healthy lifestyle choices.
  • Participate in community outreach initiatives focused on nutrition and wellness.
  • Perform additional duties as required.

Qualifications:

  • Bachelor's degree in Nutrition, Dietetics, or a related discipline.
  • Registered Dietician (RD) or Registered Dietician Nutritionist (RDN) certification from the Commission on Dietetic Registration (CDR).
  • Valid Dietician license in California.
  • Bilingual proficiency in English and Spanish.
  • Ability to work in a hybrid environment, combining remote work with in-person responsibilities.
  • A minimum of 2 years of experience in clinical nutrition, ideally within a healthcare context.
  • Experience with CalAIM or similar initiatives is preferred.
  • Strong understanding of medical nutrition therapy across various health conditions.
  • Exceptional communication and interpersonal skills.
  • Capability to work independently and collaboratively within a multidisciplinary team.
  • Proficient in electronic health records and nutrition management software.
