Position Summary:

We are looking for a committed and skilled Dietician to enhance our healthcare team. This position will focus on addressing dietary needs and consultations, particularly for CalAIM members. The Dietician will offer expert nutritional advice, create personalized meal plans, and work closely with healthcare professionals to improve patient health outcomes.

Key Responsibilities:

  • Perform thorough nutritional evaluations for patients enrolled in the CalAIM program.
  • Design, execute, and assess individualized nutrition strategies tailored to the specific needs and health conditions of patients.
  • Provide nutritional guidance and education to patients and their families, emphasizing healthy eating practices and dietary management of medical conditions.
  • Review and process all medically tailored meal referrals within the CalAIM framework.
  • Collaborate with physicians, nurses, and other healthcare professionals to formulate and implement comprehensive patient care plans.
  • Track and assess patient progress, modifying nutrition plans as needed to achieve optimal health outcomes.
  • Stay updated on the latest research and advancements in nutrition, ensuring that all recommendations and plans are grounded in current evidence-based practices.
  • Document all patient interactions, assessments, and plans in the designated medical records.
  • Contribute to the development and execution of nutrition-related policies and procedures.
  • Conduct group educational workshops and sessions on nutrition and healthy living.
  • Assist in organizing and implementing community outreach initiatives focused on nutrition and wellness.
  • Perform additional duties as required.

Qualifications:

  • Bachelor's degree in Nutrition, Dietetics, or a related discipline.
  • Registered Dietician (RD) or Registered Dietician Nutritionist (RDN) credential from the Commission on Dietetic Registration (CDR).
  • Licensed to practice as a Dietician in California.
  • Bilingual in English and Spanish.
  • Hybrid role; must be able to work remotely and attend necessary meetings.
  • Minimum of 2 years of experience in clinical nutrition, preferably within a healthcare environment.
  • Experience with CalAIM or similar programs is highly advantageous.
  • Strong understanding of medical nutrition therapy for various health conditions.
  • Excellent communication and interpersonal abilities.
  • Capability to work effectively both independently and collaboratively within a multidisciplinary team.
  • Proficient in electronic health records and nutrition software.
