Compensation is hourly and ranges from $35 - $47 / hr based on experience.
Seeking a Part-Time Clinical Case Manager

Key Responsibilities:
- Coordinate patient care from admission to post-discharge.
- Facilitate team conferences and treatment plan modifications.
- Educate patients and families on rehabilitation and the role of a Case Manager.
- Monitor compliance with regulations and make appropriate referrals.
Qualifications
- Must hold an active clinical license.
- Minimum of a bachelor's degree. Graduate degree preferred.
- 2 years of rehabilitation experience preferred.
