SUMMARY

As a Case Manager at LTLC, you will be responsible for providing comprehensive support and services to a caseload of 15-30 individuals experiencing homelessness. Your duties will include conducting needs assessments, developing individual service plans, and connecting clients with appropriate community resources and services such as housing, healthcare, employment, and mental health support. You will work collaboratively with clients to set and achieve goals, advocating on their behalf and ensuring they receive the necessary assistance to improve their overall well-being and transition to stable housing. Strong communication, empathy, and organizational skills are essential in this role, as you will play a critical part in helping clients navigate complex social systems and achieve self-sufficiency.

PRIMARY RESPONSIBILITIES

  • Plan and develop program services according to client needs
  • Participates weekly program staff and team meetings
  • Work closely with SMOC Housing Staff to ensure coordinated housing and services for each participant.
  • Prepare monthly program report to be submitted to the CM Supervisor.
  • Maintain monthly and annual statistical activity reports as requested by funding sources
  • Maintain direct contact with client population through continued service delivery and by acting as back-up for direct care staff when necessary
  • Work with staff to provide them guidance and ensure optimal delivery of services to clients
  • Develop and coordinate process for responding to verbal/behavioral disturbances that occur at the program
  • Works collaboratively with the City of Lowell CoC as directed by CM Supervisor
  • Engage all clients by understanding and addressing their needs whether within or outside the scope of the specialty.
  • Attend & participate in engagement team meetings, follow up with client service plans and communicate effectively with clients and staff in other specialty areas.
  • Ensure compliance with program/department, agency and/or funder requirements, as well as SMOC policies & procedures.
  • Other duties as assigned.

KNOWLEDGE AND SKILL REQUIREMENTS

  • Must have commitment and ability to work with others to change systems which obstruct the growth and development of homeless and disadvantaged persons.
  • Must have the ability to work with diverse populations (e.g. mentally ill, chemically dependent), diverse interests, and respond to ever-changing community dynamics.
  • Need to have strong empathy, advocacy, empowerment, teaching and problem solving skills.
  • Must have excellent verbal and written communication skills.
  • Proficient in Microsoft Word and Excel.
  • 1-3 years serving homeless population.

PHYSICAL REQUIREMENTS

  • This position requires that you be able to ascend and descend stairs.
  • Must be able to travel to various housing properties throughout Western Mass.
  • Position requires prolonged periods of sitting.

ORGANIZATIONAL RELATIONSHIP

  • Directly reports to Case Management Supervisor.
  • Direct reports of this position are none.

WORKING CONDITIONS

The program is in Shelter Property in Lowell. Parking is available in an outside lot adjacent to the building. As part of the responsibilities of this position, the case manager, Supervisor approval will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services.
