In the heart of the ongoing state of emergency due to the surge of migrants seeking shelter in Massachusetts, LCG is thrilled to partner with Commonwealth Care Alliance to present an opportunity for compassionate and dedicated care coordinators to make a significant impact in the community

We are looking to hire full time care coordinators who are passionate about being part of the solution by providing essential care to migrant families in need during this critical time.

This family care facility is located in Lowell, MA.

Job Duties/Responsibilities

  • Helping families prepare to move into permanent housing, and to connect families to a support network once they are housed
  • Assessing a family's strengths, needs and barriers to housing
  • Resolving tenant screening barriers, such as securing ID
  • Securing connections to healthcare services
  • Securing connections to job training/job placement services
  • Assistance enrolling in entitlement benefits (i.e., SNAP, TAFDC, SSI)
  • Working with parents to address educational and health needs of children.
  • Knowledgeable about service availability and eligibility and connected with key individuals in other systems including regional CoCs.

Other: Education and Experience

  • Bachelor's Degree
  • Fluent in Haitian Creole
  • Minimum two years experience with behavior management systems, treatment planning, crisis intervention, case management and providing individual, group and/or family therapy
  • Excellent written and verbal communication skills
  • Empathic understanding of patients and skilled in performing multicultural needs assessments and developing concrete service plans.
  • Knowledge of State Systems and local child and family advocate agencies
  • Computer literate including Microsoft Office and Internet Explorer with the ability to learn new software applications
  • Ability to work in a fast-paced environment, inclusive of organizational skills, critical thinking, and problem solving abilities

Hours:

  • 9AM - 5PM - Monday-Friday

Job Type: Full-time

Pay: $30.00 per hour
