Home Health Speech Language Pathologist

Expert Healthcare is in search of a skilled and compassionate Home Health Speech Language Pathologist (SLP) to join our dedicated team. In this critical role, you will provide individualized therapy services to patients in the comfort of their homes, helping them recover and improve their communication, swallowing, and cognitive-communication skills following illness, injury, or surgery. You will assess patients' needs, develop tailored treatment plans, and collaborate with caregivers and healthcare professionals to ensure optimal outcomes. - Conduct comprehensive speech and language assessments to evaluate patients' needs.
- Develop, implement, and modify individualized treatment plans based on assessment results and patient progress.
- Provide therapy services to improve communication and swallowing abilities in patients' homes.
- Educate patients and their families on strategies to enhance communication and functional abilities.
- Document patient evaluations, treatment progress, and outcomes in compliance with regulatory standards.
- Collaborate with other healthcare professionals involved in patient care to ensure a holistic approach to treatment.
Requirements
- Current and valid Speech Language Pathologist license in the state of practice.
- Degree in Speech Language Pathology from an accredited program.
- 1 year experience in home health setting required.
- Oasis charting experience required. HomeCare HomeBase preferred.
- Strong assessment and treatment planning skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a diverse caseload.
- Basic Life Support (BLS) certification is required.
