Job Summary

We are seeking a skilled Home Health Speech Language Pathologist (SLP) to join our team. The ideal candidate will have 1 year of experience in a home health setting and strong assessment and treatment planning skills.

Key Responsibilities

  • Assess patients' needs and develop individualized treatment plans.
  • Provide therapy services to improve communication and swallowing abilities.
  • Collaborate with other healthcare professionals to ensure optimal outcomes.

Requirements

  • Current and valid Speech Language Pathologist license.
  • Degree in Speech Language Pathology from an accredited program.
  • 1 year experience in home health setting.
  • Oasis charting experience required.
