How you can make a bigger difference: PURPOSE: The Supervised Independent Living Program (SILP) provides a transitional living environment for older youth in foster care who are ready to move towards independent living but could benefit from a supported environment to build the skills required to live independently. The SILP provides resources for youth aged 18-20 who are experiencing challenges as they transition to adulthood. All youth are referred to us through ACS. Our residents have experienced significant trauma, resulting in behavioral and emotional issues. The program provides physical and emotional support, family connections and visitation, educational, social and clinical support and opportunities, age appropriate recreational activities, and aftercare planning for youth and young adults. POSITION OVERVIEW: The Behavioral Health Clinician is a key member of the therapeutic team. He/She/They are responsible for supplying necessary, culturally sensitive clinical interventions within the realm of the program. The Behavioral Health Clinician works closely with the director, case manager, medical staff and programming manager to ensure appropriate evaluations and treatment interventions for the youth. KEY ESSENTIAL FUNCTIONS (List the functions that are essential to achieve the goals): Meet with assigned residents for weekly sessions, engage and remain in contact with their parents, ensure home visits are conducted, vocational/ Educational, psychiatric, recreational services are provided. Identifying and providing access to specific concrete services and community resources Engage families in planning for service Identifying and providing access to specific concrete services and community services Advocate, as necessary, with external resources such as mental health providers, treatment facility, community members etc. on behalf of the residents Provide individual and family counseling both on-site and through telehealth Provide support to youth who identify as LGBTQAI+. Liaise between program, family court, ACS, case planners, youth's family members, and relevant community members. Incorporate DBT skill building into work with youth and their families. Attend other planned ACS and school conferences, as well as court hearings. TEAM PROCESS: Participate in weekly team meetings, and may lead the meeting on occasion. Prepare integrative treatment plan, based on all assessments from the other professionals. Ensure follow-up as necessary from meetings. Conduct meetings to ensure proper treatment, progress in program and coordination of goals. Provide psychoeducation and intervention support to Direct Support Professionals. Work with Direct Support Professionals to enhance communication and cooperation between them and consumer families. Provide crisis intervention, as needed. Other duties as assigned. DOCUMENTATION: Complete paperwork, including case notes, FASPs, PHR's and court reports, in a timely manner. Complete psychosocial assessment within the first 30 days of youth's admission. Participate in audit preparation. CORE COMPETENCIES: Commitment to recovery-oriented trauma-informed practice Embraces youth-guided family-driven care Promotes teamwork Takes initiative Liaise between department staff and middle managers/administration Good interpersonal communication and collaboration skills Solution Focused Uses Data to Inform & Guide Practice Values and Demonstrates Cultural Competency Values professional development and participation in clinical supervision and training EDUCATIONAL/TRAINING REQUIREMENTS/ LANGUAGE PREFERENCE (List what the desire to reach goals): License Masters in Social Work (LMSW), License Marriage and Family Therapist (LMFT), Licensed Mental Health Counselor (LMHC) or Licensed Creative Arts Therapist (LCAT) with a minimum of one year of relevant experience with similar population, LCSW, PhD. Valid Driver's License required. Spanish speaking a plus. COMPUTER SKILLS REQUIREMENTS: Knowledge of electronic Health record systems or CONNECTIONS preferred. Working knowledge of Microsoft Office, Outlook and Word. Aptitude for learning new systems and application software Experience with documenting in electronic health records and using Microsoft Office software. Experience with telemental health platforms a plus. WORK ENVIORNMENT/PHYSICAL EFFORT: Travel in the community with residents using public transportation , a cab, Uber etc. Engage in physical activities with the youth. Walk up and down stairs. VISUAL AND MANUAL DEXIERITY: The candidate should be able to read paper and electronic documents and perform significant data entry into various computer programs.
