The Licensed Social Worker will provide diagnostic assessments, therapy, counseling, and supportive services for a diverse population experiencing persistent and chronic mental illness. This role requires a strong understanding of social determinants of health and the ability to develop patient-centered treatment plans.
Responsibilities:
• Conduct diagnostic assessments and develop treatment plans for patients with mental health and social issues.
• Provide individual, group, and family therapy sessions to patients.
• Collaborate with external providers for evaluations and coordinate care with mental health and social service contacts.
• Maintain professional relationships with staff and community agencies.
• Meet daily service goals and manage a caseload of approximately 120 patients.
Requirements and Qualifications:
• LMSW, LCSW, LMHC, or LMFT licensure in New York State.
• Minimum one-year experience working in an outpatient behavioral health clinic.
• Excellent organization and communication skills.
• Ability to prioritize multiple tasks efficiently and develop patient-centered treatment plans.
• Bilingual Spanish speaking required.
