The Frontline Emergency Assistance Case Manager provides case management and crisis intervention as needed around basic needs including food, clothing and utility assistance through financial assistance, goods, advocacy and referral to relevant resources to foster self-sufficiency.

Position Title and Department:

Pay: $46,320/year ($1,000 additional per year or hour for bilingual capability in English and Spanish, French or Arabic and $1,000 additional per year for additional degree of value to ACTS.)

Reports to: Director of Housing Services

Hours per week and Schedule: 40 hours; Monday to Friday, MWR 9 a.m. - 5 p.m., T 9:00 a.m. - 7:00 p.m. and F 9:00 a.m. - 3:00 p.m.

Essential Functions:

  1. Conducts assessment for crisis walk-ins and calls to determine eligibility for program services including supplemental food assessment, weatherization, agency sponsored utility assistance and/or thrift store clothing voucher.
  2. Provides case management, assisting clients with the development of Individualized Service Plans, supports clients with accessing resources and provides follow up to ensure client stability and success.
  3. Makes referrals to other services and with other community providers based on eligibility and appropriateness.
  4. Executes assigned program funding within the approved budget and in compliance with funding source requirements.
  5. Advocates for clients with community agencies, systems and services and serves as liaison for same.
  6. Enters and updates information in program specific databases.
  7. Accurately completes required paperwork to ensure timely payment of client bills.
  8. Maintains client files/database in accordance with ACTS’ and accrediting agency guidelines.
  9. Actively participates in supervision, attends department meetings, community meetings, training and other agency related activities.
  10. Completes required training annually including CPR, First Aid, NARCAN certification and other human service trainings.
  11. Performs other duties as assigned.
Required Knowledge, Skills and Abilities:
  1. Strong oral and written communication skills

  3. Ability to function independently and as part of a team
  4. Ability to multi-task in a fast-paced work environment
  5. Ability to respond in a stressful situation in a flexible and professional manner
  6. Working knowledge of MS Word, Excel, Outlook, and Internet navigation

Education and Experience:
  • Meets one of the following three criteria:
    • A bachelor’s degree in human services, behavioral sciences or allied field (including criminal justice, education and psychology)
    • AA degree in human services, behavior sciences or allied field (including criminal justice, education and psychology) with 4 years of documented related experience
    • No college degree in human services, behavioral sciences or allied field (including criminal justice, education and psychology) with 5 years of documented related experience.
  • A bachelor’s in human services or behavioral sciences, preferred
  • Preferred – CPR and First AID Certification
Physical Requirements:
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
