Come join our Individual Supports team today Full time or Part Time $19.00 per hour GENERAL SUMMARY Provides instruction, supports and positive role modeling to individuals with varying developmental disabilities and/or acquired brain injury living in their own homes. Encourages the development of life, social, and recreational skills, as well as, community access and healthcare. Provides direct, active and constructive interactions with individuals in cooperation with staff, team members, clinical consultants, community members, families and managers. ESSENTIAL JOB FUNCTIONS * Provides training in personal care to individuals who require support. Implements individual service plan (ISP) goals and objectives as defined on established service agreements. Prepares and forwards all required ISP paperwork and reports according to established procedures and deadlines. Maintains individual program and medical records in compliance with Agency and regulatory standards. Ensures individuals receive appropriate medical and therapeutic care; maintains corresponding documentation. Reports all incidents in a timely fashion and in accordance with Agency and regulatory standards. Maintains regular, open lines of communication and a positive rapport and proactive attitude with staff, families, supervisors, and Department of Developmental Services (DDS) and Massachusetts Rehabilitation Commission (MRC) representatives. Provides training in meal preparation to ensure meals are nutritious and in compliance with physician’s dietary orders; trains individuals in the areas of menu planning, grocery shopping, budgeting and bill paying. Teaches individuals how to maintain a clean and sanitary household. Assists individuals with attending medical appointments; ensures health care practitioners complete required documentation. Provides teaching and adaptive supports to individuals regarding medication administration. Provides assistance and support to individuals with scheduling and attending social and leisure activities. With certification, administers medications within full compliance of state and Agency guidelines; ensures residence is in compliance with Department of Public Health (DPH) Medication Administration Program (MAP). Assists individuals with attending religious faith workshops or services. Attends required meetings and trainings. Provides transportation to individuals to appointments and social activities utilizing the Agency assigned vehicle, when available. Adheres to the Agency’s emergency response policies and procedures. Provides rotational on-call support, as needed. Ensures basic human rights, well-being and safety of individuals is respected at all times. Serves as mandated reporter for suspected cases of abuse and/or neglect to the Disabled Persons Protection Commission (DPPC) or Elder Abuse Hotline, as appropriate. OTHER DUTIES AND RESPONSIBILITIES Performs additional duties, as assigned. Prior human services or relevant life experience preferred of at least 2 years. Valid driver’s license in the state of Massachusetts. Successful completion of all state and Agency mandated trainings. Successful completion of Medication Administration Training certification. SUPERVISORY RESPONSIBILITY No supervisory responsibility. WORKING CONDITIONS/PHYSICAL DEMANDS Frequent standing, walking and bending. Ability to lift up to 50 pounds. Ability to provide safe transportation including driving for extended periods of time.
