Clinical Pharmacist (Night Shift)

Position Duties and Responsibilities:
  • Assess patient’s response to drug therapy and planning drug therapy based on physician-established diagnoses.
  • Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes.
  • Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in cooperation with the medical staff.
  • Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotic).
  • Administer prescription or non-prescription drugs according to established agreements or MTF protocols.
  • Assess metabolic needs and order therapeutic enteral or parenteral nutrition products in the inpatient setting.
  • Evaluate medical and medication histories for drug-related problems and adjusting drug therapy accordingly.
  • Compound medications as required.
  • Consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options.
  • Participate in patient rounds with clinicians to discuss proper patient medications and dosing.
  • Counsel patients regarding medications, including patients who are discharging.
  • Monitor and manage medication dispensing units (i.e., Pyxis) for appropriate removal, replenishment, and clinical review.
  • Provide guidance in dispensing medications, making purchasing decision, verifying calculations, and preparing IV medication, and overseeing drug administration.
  • Respond to, and participate in, any facility emergency codes as required by the facility.
  • Perform a full range of pharmacist procedures.
  • Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
  • Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients.
  • Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
  • Provide medication and healthcare related in service training to clinical staff on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head.
  • Consult with other specialty practitioners to provide optimal patient pharmaceutical care.
  • Work closely with clinical staff to develop and support patient wellness programs as directed by DHA (i.e., smoking cessation, asthma, and diabetes classes).
  • Prepare all documentation to meet or exceed established MTF standards, to include, but not limited to timeliness, accuracy, content, and signature. Contractor employees shall only use MTF and Air Force-approved abbreviations for documentation in the patient health records.
  • Document significant medication interactions, and pharmacy interactions with prescribers. Conduct Adverse Drug Reaction (ADR) reporting in accordance with The Joint Commission (TJC) or the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC) standards as appropriate.
  • Facilitate medical staff drug utilization reviews (DURs) as deemed necessary.
  • The Pharmacist's productivity is expected to be comparable to that of others assigned to the same position and authorized the same scope of care, with minimal supervisory direction. The contracted employee shall maintain a level of productivity and quality consistent with: complexity of the assignment; facility policies and guidelines; established principles, ethics, and standards of practice of professional pharmacists, The Joint Commission, Unit Effectiveness Inspection, Florida State Law and all applicable Federal Regulations along with other applicable DoD and service specific guidance and policies. Shall also comply with the Equal Employment Opportunity (EEO) Program, infection control and safety policies and procedures.
Minimum Qualifications:
  • Successful completion and graduation with Doctor of Pharmacy (Pharm.D.) from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
  • Current American Heart Association or the American Red Cross Advanced Life Support (Adult) certification.
  • Shall have at least 12 months of inpatient pharmacy experience within the previous 36 months practicing as a pharmacist as well as at least one of the following 3 options: Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy. OR Board Certification in one or more of the pharmacy specialties recognized by the Board of Pharmaceutical Specialties. OR Documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.
  • Shall have and maintain a current, full, active, and unrestricted license to practice as a pharmacist in any one of the 50 states, the District of Columbia, the Commonwealth of Puerto Rico, or the U.S. Virgin Islands.
  • Must be a US Citizen.
