Position Overview:
GiaCare Inc. is seeking qualified and experienced civilian Pharmacists to support healthcare operations. This role is essential in providing pharmaceutical care and ensuring optimal medication management for patients.

Essential Qualifications:
Educational Background: Doctor of Pharmacy (Pharm.D.) from an ACPE accredited institution.
Internship/Residency: Completion of a clinical pharmacy residency or fellowship accredited by recognized pharmacy organizations, or equivalent training and education in clinical pharmacy.

Certifications:
Basic Life Support (BLS) certification from a recognized provider.

Experience:
A minimum of one year of experience in a hospital pharmacy setting, with proficiency in unit-dose distribution and IV admixtures.

Licensure:
Must hold a current, unrestricted pharmacist license from any U.S. state or territory.

Key Responsibilities:
As a pharmacist, you will:

  • Evaluate and manage patient medication therapies based on established diagnoses.
  • Order and interpret laboratory tests to assess the effectiveness of drug therapies.
  • Adjust medication regimens for chronic conditions in collaboration with healthcare providers.
  • Monitor pharmacotherapy for necessary adjustments based on patient-specific factors.
  • Dispense medications according to established protocols.
  • Assess nutritional needs and recommend appropriate enteral or parenteral nutrition.
  • Review patient medication histories to identify and resolve drug-related issues.
  • Collaborate with multidisciplinary teams to optimize patient care.
  • Conduct thorough medication reviews during dispensing processes.
  • Ensure the accuracy and appropriateness of all medications dispensed.
  • Provide training to clinical staff on new medications and therapeutic options.
  • Document significant drug interactions and maintain communication with prescribers.
  • Participate in drug utilization reviews as needed.
