GiaCare Inc. is seeking qualified and experienced civilian Pharmacists to support a significant healthcare initiative. This role is integral to the operations of a military medical facility, focusing on delivering high-quality pharmaceutical care.
Key Responsibilities:
- Evaluate and manage patient responses to medication therapies based on established medical diagnoses.
- Order and interpret laboratory tests to assess the effectiveness of drug therapies.
- Initiate, adjust, or discontinue medications for chronic conditions in collaboration with healthcare professionals.
- Monitor pharmacotherapy regimens that require ongoing adjustments due to pharmacokinetic variations.
- Administer medications in accordance with established protocols.
- Assess nutritional needs and prescribe appropriate enteral or parenteral nutrition solutions in inpatient settings.
- Review patient medication histories to identify and resolve drug-related issues.
- Collaborate with a multidisciplinary team to optimize patient care.
- Conduct comprehensive pharmacist procedures to ensure quality and safety in medication dispensing.
- Review and verify prescription details to ensure accuracy and appropriateness for individual patients.
- Oversee the preparation and processing of pharmaceutical products by technical staff.
- Provide educational training to clinical staff regarding new medications and their appropriate use.
- Document significant drug interactions and maintain communication with prescribers regarding patient care.
- Facilitate drug utilization reviews as necessary.
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Completion of a clinical pharmacy residency or equivalent experience.
- Current, unrestricted pharmacist license from any U.S. state or territory.
- Basic Life Support (BLS) certification.
- Minimum of one year of experience in a hospital pharmacy setting.
