Case Manager- Addictions Recovery

Purpose:

The Case Manager will understand, communicate and promote the Addictions Recovery Program's policies, procedures and objectives. The Case Manager will provide consistent supervision of all clients in the facility, ensuring their safety, progress in recovery, abstinence from drugs and alcohol, and compliance with program policies. The Case Manager is expected to understand the disease of addiction, be familiar with each client's individual issues, and support their progress in recovery.

  • Adhere to established policies and procedures.
  • Contribute to fund development efforts for support of all programs and the overall Association.
  • Provide support to other departments and staff.
Essential Duties:
  • Listen and assist clients with options as client issues arise; create and maintain high quality work environment.
  • Participate in weekly staffing conferences to evaluate and monitor each client's progress and coordinate care.
  • Regularly review charting and communication notes about clients' drug screen results, medication compliance and progress in recovery services. Maintain complete and organized client files.
  • Accurately track and maintain client data required to track outcomes for program funding. Prepare thorough reports on client progress as required by referral and funding sources.
  • Maintain confidentiality of all client information, as required by state and federal law.
  • Review, facilitate, and assist with client weekly progress by addressing areas noted on Weekly Assessment form.
  • Consistently maintain and oversee client compliance with House Rules, and direct appropriate disciplinary action.
  • Comply with building security procedures and incident reporting protocol.
  • Calmly and assertively address client emergencies, sickness, or relapse. Connect clients with resources to address their needs.
  • Assist with preparing client records and documentation, including Treatment Plans, Reviews, and Discharge procedures.
  • Attend all staff meetings.
  • Assist with all in-house meetings while on duty, including Community Meeting, PLUS Meeting, Women in Recovery, Women's Independence Project and oversee clients during other support group meetings.
  • Participate in On-Call as required.
  • Collaborate with YWCA Addictions Recovery Program staff and clientele staff, and external providers and referral sources to coordinate high-quality client care.
  • Comply with state standards.
  • Participate in program marketing and outreach efforts.
  • Process client intakes and discharges.
  • Transport clients in YWCA NEIN vehicles as needed.
  • Other duties as assigned to meet the organization's goals.
Requirements

Education:
  • Bachelor's in Social Work, Psychology, Human Services or other related fields
  • CPR and First Aid Certifications
Knowledge and Experience:
  • Experience in addiction recovery.
  • Proven ability to effectively and respectfully interact with diverse population, including clients, co-workers and external stakeholders.
  • Experience in crisis intervention, networking, team building and public speaking.
  • Strong customer service skills with ability to communicate in a professional manner with a wide range of people.
  • Ability to exercise independent judgment and discretion when handling confidential or sensitive manners.
  • Strong time management skills, ability to take initiative to solve problems and work with minimum supervision.
  • Proficient in Microsoft Office including Outlook, Word and Excel.
  • 1-3 years of general office experience, including reception and telephone duties.
  • 1-3 years of experience with general office equipment (copiers, printers).
  • Ability to read, write and speak English.
  • Accurate, organized, and detail-oriented, with ability to work under pressure and shift priorities in response to changing needs.
  • Ability to keep accurate records and files.
  • Must comply with organization's policy of automobile insurance: must have a valid driver's license and personal automobile insurance coverage.
  • Flexibility to change work hours as needed.
Physical/Mental Essential Requirements:
  • Walking, standing, bending, stooping, reaching, moderate lifting and carrying (up to thirty (30) pounds).
  • Full range of body motion including manual and finger dexterity and eye-hand coordination.
  • Requires corrected vision and hearing to normal range.
  • Ability to sit at a computer work station for extended periods of time.
  • Occasional need to stand for long periods of time.
  • Ability to focus on detail and accuracy of work product.
  • Work is performed primarily in a non-smoking office environment, though travel may expose employee to inclement weather conditions.
  • Occasional high stress may be experienced in dealing with clients, staff and volunteers.
