Summary of work:

The SUD Care
Coordinator provides care coordination for participants struggling with drug
and/or alcohol addiction, particularly those receiving Medication Assisted
Treatment (MAT) for opioid addiction. This employee is a core member of our
Balance Works team and works closely alongside MAT physicians, Integrated
Behavioral Health, and Social Services staff. The Care Coordinator works with the
local FATOS and System of Care to link participants to the appropriate level of
care, as well as to other recovery supports within LSSI and the community.

Responsible for
conducting required screenings and assessments, writing case plans and providing
case management and care coordination services according to department needs,
guidelines, and procedures. Develops a supportive relationship with individuals
and families and provides consultation and care coordination in schools,
hospitals, home visits, and/or other locations as deemed necessary. Provides connection and referrals to
individuals and families in need of supportive services, and completes
appropriate documentation within established timeframes.

Essential duties
and responsibilities:

· Effectively engages individuals and families
in the therapeutic process and completes mental health/substance abuse
assessments as appropriate utilizing a systems framework and trauma informed
approach among other modalities.

· Works independently and consistently
exercises discretion and sound judgment in providing short and long-term case
coordination/counseling services as well as stabilization and support services
to individuals and families needing specialized intensive care coordination and
behavioral health services. Meets programmatic benchmarks for productivity.

· Respect all hospital, school, and location
policies and procedures when conducting screening, assessments, appointments,
and follow up visits.

· Ability to deliver services in the office,
home and community based settings and adapt to the unique requirements of each
setting.

· Work with multidisciplinary to provide
coordinated services.

  • Responsible
    for thorough, timely and accurately written documentation for the client file.
    Records all necessary information in the prescribed data systems.
    Completes statistical information in a timely manner for agency and funder
    purposes as required.
  • Complete
    paperwork and follow agency procedures and accreditation guidelines while
    performing work duties. Help with reaccreditation process when assigned.

· Ensures all insurance and contract billing
for client services is recorded properly and follows protocol and rules for
collection of co-pays, deductible and subsidized fees. Ensures appropriate
documents are verified for billing purposes and information is submitted
according to program protocol and agency policy.

· May provide group therapy, lead support
groups, conduct psychosocial educational groups, and provide presentations to
community groups or consultations to schools and secondary settings. May
provide recommendations and testimony for court (specifically when working with
DCS involved clients).

· Attend department team/staff meetings and
present cases, as requested.

· Assist in providing referral information to
families and connects individual /family to additional services as needed if
the program is full, or they do not meet the minimum program requirements at
LSSI.

· Serve as a client advocate and respect all
agency, program and meeting location confidentiality policies.

· Offer and provide evidenced based curriculum
and education to clients/parents when appropriate.

· Meet with Supervisor on a regular basis and
consult with Supervisor on assigned cases.

· Report any case that may involve “imminent
harm” or “threatened harm” to a supervisor immediately. Always intervene on behalf of the child when
necessary.

· Report to Child Protection Services in cases
where neglect and/or abuse is suspected.

· Attend weekly staff meetings, in-service
training and other meetings as requested.

· Attend program related meetings in the
community, as requested.

· Complete all paperwork required by agency
within established program timeframe.

· Track balanced score card goals and turn in
required reports to the program manager.

· Provide assistance in other programs,
counties and regions when necessary.

· Perform other related duties as assigned.

· Develop and maintain working relationships
with community partners and collaboration.

· Participate in grant writing and subsequent
reporting for programs.

· Duties may include assisting director with training
of new staff.

· Participate in interviews with director as
needed.

and families.

Education and
Experience:

· Bachelor’s degree in Social Services or
related field required and two years’ experience preferred.

· Unencumbered Licensed Addiction Counselor
(LAC) in the State of Indiana preferred, LAC eligible accepted.

· Experience in screening, assessments,
treatment and recovery strategies, use of MAT; Motivational Interviewing, and
strategies for working with child welfare agencies, correctional institutions
and other health and social services agencies that serve individuals with
substance use disorders.

· Valid driver's license and car for full-time
business use.

· Must have access to the internet.

· Experience with computers.
