Description

Reporting to the Manager of Operations, provides support of project data collection and performance assessment as well as implementation of the evaluation plan. Coordinates activities with the Manager of Operations and shares timely data/communication as needed.

Position Type:Full-Time/Regular

Essential/Primary Duties:
  • Assist with implementation and tracking for local evaluation
  • Provide timely collection of TRAC/NOMS and local evaluation to Internal Evaluation Specialist
  • Complete data entry of evaluation data
  • Supports the goals of the agencies mission
  • Follow all confidentiality/HIPAA guidelines
  • Other duties as assigned by Supervisor
  • Track admissions into the residential program and determine client eligibility for grant purposes.
  • Admit client into grants as eligible and track clients throughout continuum of care
  • Gather GPRA data and other data sets on clients at admission and discharge.
  • Follow up with clients to conduct a 6 month follow up interview
  • Data enter all data sets including GPRA into the SPARS system, additional questionnaires into Access databases at admission, discharge, and follow up.
  • Track services provided and enter data on corresponding forms on an ongoing basis
  • Provide weekly admission numbers to Director and monthly admission numbers to external evaluation consultants (IMPACT)
  • Follow up with residential clients on a monthly basis to conduct monthly mini interviews and other contracted surveys
  • Collect updated contact information from clients during the monthly interview
  • Work on community improvement projects throughout the year including attending meetings. Create reports to present at various meetings and community events
  • Prepare expense vouchers in order to request incentives for grants
  • Obtain gift cards to use for incentives
  • Track training of all HSD staff on a monthly basis
  • Provide data reports for Quarterly and Annual reports including gathering data from other staff
  • Collect client success stories and prepare for annual reports


Qualifications

Bachelor's degree, course work in social science research. Experience in data entry and report writing support. Experience with human services populations. Experience working with a Latino Population. Computer Skills, including familiarity with major database programs. Ability to work with numbers/ quantitative data. Oral and written communication skills. Stay current on all public/private grant outcome measurement requirements. Excellent personal and professional boundaries and problem-solving skills. Bilingual preferred: English- Spanish. Understanding of cultural competencies for working with Latino adults. Clinical experience related to mental health or older adults preferred.
