Description

The Program Department working with the Community Health Corps Department compassionately advocates for residents of Detroit who need us most by providing tools to navigate barriers, accomplish goals, and move towards self-sufficiency. The Community Health Corps staff mentor, motivate, and connect Detroit residents to services and support including employment and training opportunities, food security assistance, rental assistance, and behavioral health services.

Under supervision of the Program Director, the Lead Case Manager - CDC manages a team of Case Managers and Community Health Workers (CHWs) who remove barriers for Detroiters. This position is a member of the department leadership team and serves as a leader by supervising, supporting, and motivating their team. The Lead Case Manager is accountable for accurate and complete case management data and record keeping.

Requirements

CORE DUTIES AND RESPONSIBILITIES

  • Oversee an internal team of Case Managers and CHWs who possess a range of expertise, talents, and lived experiences.
  • Develop and implement plans, procedures, services, systems, and data tracking mechanisms to assess the needs and barriers of Detroit residents and identify tactics and resources to address those needs.
  • Implement a monitoring system to track the progress of clients, including the presence or absence of barriers, and make adjustments as needed.
  • Supervise day-to-day work of team.
  • Provide coaching, mentoring, and guidance to direct reports including facilitating weekly one-on-one meetings; setting measurable and attainable goals; evaluating employee performance outcomes and metrics and taking appropriate action; and reviewing and approving timecards.
  • Develop, maintain, and expand working knowledge of relevant social services agencies, resources, and research to ensure fruitful partnerships and enhance client access to appropriate services.
  • Provide approval for resource disbursement to participants.
  • Coordinate and collaborate with City of Detroit Departments on resources and direct services that advance the client towards meeting their defined goals of success.
  • Develop, maintain, and modify metrics to quantify the impact of the team's work.
  • Develop and provide training opportunities based on staff needs and skillsets.
  • Collaborate with data team to ensure data accuracy.
  • Complete grant reports and progress updates, as needed. Participate in regular meetings with funders.
  • Participate in and contribute to regular leadership team meetings and activities.
  • Monitor and evaluate the program's effectiveness, investigate trends, and recommend and implement modifications to improve program effectiveness.
  • Perform other related duties as required.
REQUIRED QUALIFICATIONS
  • Bachelor's degree from an accredited college or university in Social Work or relevant field
  • Minimum five (5) years of equivalent experience in the field of Human Services, Advocacy, and/or Case Management in a client service setting
  • Minimum two (2) years of supervisory experience in a relevant field
  • Experience managing a team and working with staff with varied skillsets and life experiences
  • Experience working with people in crisis and conflict, including those facing homelessness, financial challenges, mental illness, and/or chemical dependency
  • Strong sense of service to Detroit residents
  • Strong creative and independent thinking skills
  • Demonstrated understanding of social work practices
  • Demonstrated understanding of social service programming
  • Ability to adhere at all times to strict confidential, ethical, and legal standards
  • Highly organized; able to prioritize and re-prioritize (when necessary) multiple tasks and projects, and meet deadlines
  • Ability to exhibit and maintain professionalism, including when under pressure
  • Ability to exercise discretion and good judgement, particularly when handling confidential information
  • Ability to communicate clearly and professionally verbally and in writing, can prepare timely, accurate, and complete reports and presentations.
  • Proficient in Microsoft Word, Excel, and ability to quickly learn client data systems
  • Valid State of Michigan Driver's License (position requires field work)
  • Successful completion of background check and drug screen
DESIRED QUALIFICATIONS
  • Master's degree
  • Valid Licensed Master Social Worker (LMSW) or Limited Master's Social Work (LLSW) certification

TERM

This is a grant-funded position anticipated through November 2024 with a possibility for longer term renewal with other funding sources.

PHYSICAL DEMANDS

The employee generally engages in light physical activity with person to person contact inside and outside the office environment. The employee is expected to move about to accomplish tasks. Must be able to lift, push, pull, and carry up to 20 pounds.

WORK ENVIRONMENT

Work is performed in indoor and outdoor environments (e.g., home visits, office, care center, clinic. etc.). Work requires travel to various homes and businesses in all areas within the City of Detroit.
