Job Overview:

Key Responsibilities:

- Provide expert pharmacy consultation services to healthcare organizations, patients, and other stakeholders

- Develop and implement medication management strategies to optimize patient outcomes

- Conduct medication therapy reviews to identify and resolve medication-related problems

- Provide patient education on medication use, side effects, and interactions

- Collaborate with healthcare teams to develop and implement medication-related policies and procedures

- Stay up-to-date on the latest developments in pharmacy practice, including new medications, drug interactions, and regulatory changes

- Develop and deliver education and training programs for healthcare professionals and patients

- Provide guidance and support to pharmacy staff on medication management and dispensing practices

Qualifications:

- Doctor of Pharmacy (PharmD) degree from an accredited institution

- Current pharmacist licensure in the state of practice

- Strong clinical knowledge and expertise in medication management

- Excellent communication and interpersonal skills, with the ability to effectively communicate with healthcare teams and patients

- Strong analytical and problem-solving skills

- Ability to work independently and in a team environment

- Experience with medication therapy management software and electronic health records (EHRs)

- Flexibility to travel as needed
