As a CCM MA, we help clients deliver continuous care coordination by assisting chronically ill Medicare patients and providing them with monthly coordination of services. Responsibilities include but are not limited to You will assist with a panel of approximately 200 chronically ill patients to coordinate care monthly. Conducting monthly phone calls with patients, practices, pharmacies, etc. to ensure that your patients are getting what they need between office visits. You will be spending 6-8 hours on the phone daily. Creating and/or revising with the guidance of the CCM LPN a personalized care plan for each patient you call on, in coordination with the practices and providers you support. Working to identify social determinants of health, gaps in care, and eligibility for assistance or other referral services for each of your patients. You will coordinate and collaborate with the office staff, other community programs and resources to address all these effectively. Empower patients to take charge of their own wellness and goals in the context of their care plan. Making approximately 30-50 phone calls a day to yield the 12-16 patient interactions necessary to complete 20 minutes of CCM service for the month. Doing all the above while maintaining your patients' privacy, confidentiality and safety, and adhering to ethical, legal, and accreditation/regulatory standards always. Reviews patient medical records prior to patient visits to obtain relevant information for outreach call. Sends certain referrals and orders (e.g. imaging tests) to outside health care organizations as necessary, and tracks follow up. Manages patient records using electronic health records and completes appropriate patient charting. Initiates and maintains correspondence with patients and insurance companies as needed. Performs all other duties as required. Requirements: 1-2 years healthcare experience as a Certified/Registered Medical Assistant with active accreditation required. Technology experience - preferably with multiple EMR/EHR systems, familiar with Microsoft Office, Google Suite products and comfortable learning new software’s quickly. You will be the voice of our company, on the frontline speaking with the patients, you must be: highly professional on the phone and able to maintain a compassionate and engaging tone. Quick thinking to respond in the moment to patient questions and needs, with strong critical thinking skills to ask appropriate follow-up questions and prioritize what you learn from patients. Solid writing skills, including accurate spelling and grammar. Not only will you talk to people on the phone, but you’ll also communicate with patients and providers in writing as well. Excellent verbal skills are essential as you’ll need to explain our services with confidence, speak clearly, confidently, and have a friendly phone demeanor. You must be highly self-motivated, organized and able to work independently. Call Center and/or Triage experience is a plus. Case management or care coordination experience is a plus Qualifications Education: High School Diploma/GED required. Graduated from a Certified Medical Assistant Program (Certificate from an accredited school for Medical Assistants); current CPR certification from the American Heart Association are required. Bi-lingual English/Spanish a plus. Experience: 1-2 years’ experience as a Certified Medical Assistant is required. Previous medical office experience as a medical/nursing assistant is strongly preferred. Knowledge/Abilities: All skills of a Medical Assistant must be met. Must possess strong communication and interpersonal skills with an emphasis on excellent customer service when dealing with both medical staff and patients. Computer skills are required with the ability to learn Centricity EMR and Outlook. CS-HHC is an equal opportunity employer.
