JOB SUMMARY

Responsible as a member of the interdisciplinary team to the nurse care team leaders for assigned functions related to providing information about services to potential John Knox Village Hospice patients and admitting patients. Assesses, interviews and develops a plan of care. Assists with continuity of care and discharge planning in hospitals. Follows admissions, ensure smooth transition of new patients to care manager. Supportive of the Village’s mission, values, and service/hospitality (PRIDE) expectations. Completes training as required by policy.

ESSENTIAL JOB FUNCTIONS

  • Determine patient’s status and appropriateness for JKV hospice services per JKV service policies and admission criteria. If inappropriate for Hospice, explain JKV’s array of health services.
  • Plan for admission of patients to JKV Hospice in coordination with nurse care team leaders.
  • Assume initial responsibility for assessing patient/family needs and consulting with attending physician and other hospice staff members.
  • Admit patients, develop plan of care and complete related paperwork, per JKV policies and procedures.
  • Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
  • Assist with implementation and coordination of services required for discharge of patient from hospitals into JKV’s program of care.
  • Facilitate transition of patient/family already in care to primary nurse at appropriate time.
  • Instruct primary care givers, volunteers and employed caregivers to provide care as indicated.
  • Inform the nurse care team leaders of unusual or potentially problematic patient/family issues.
  • Attend nursing reports and the IDT conference to relay information to the IDT about new patients and to ensure continuity of JKV’s patient care service as indicated.
  • Manage patient/family care plan and accompanying services between time of admission and transition to nurse care manager.
  • Screen and process inquiries for admission to JKV Hospice.
  • Participate in JKV’s orientation and in-service training programs for professional staff.
  • Participate in agency and community programs as requested to promote professional growth and understanding of hospice care.
  • Demonstrate familiarity with policies of the agency and rules and regulations of state and federal bodies, which aid in determining policies.
  • Act as visit nurse providing assistance to care team leaders as assigned by clinical supervision.
  • Assists with JKV’s QAPI program as needed.
  • Conduct audits, reviews, and participates in the agency annual review as assigned.
  • Adheres to all safety regulations and requirements.
  • Handles stress well and gets along with others within the workplace and with customers outside the department.
  • Maintains acceptable standards of workplace conduct.
The responsibilities described above represent the primary responsibilities of the job. Other responsibilities may be assigned by the supervisor as warranted by business needs. The incumbent is expected to perform all assigned responsibilities.

ESSENTIAL QUALIFICATIONS

Education:
  • This job requires a combination of education, training and experience that is acquired through the completion of an accredited School of Nursing. Must be licensed as RN in Missouri. Incumbent may hold a multi-state license from any other nurse licensure compact state that allows them to practice in the state of Missouri.
Licenses/Certifications:
  • Certified Hospice Palliative Nurse (CHPN) preferred.
  • Current Basic Life Support (BLS) certification or ability to obtain prior to direct patient contact.
  • Valid driver’s license and proof of auto insurance.
Knowledge and Skills (required unless otherwise noted):
  • Strong telephone interviewing, assessment, communication, and negotiation skills.
  • Ability to work in a field setting and exhibited ability to make sound nursing judgments.
  • Knowledge of physical, psychosocial and spiritual needs of terminally ill patients and their caregivers.
  • Must have demonstrated knowledge and skills necessary to provide care to and communicate with primarily the geriatric population.
  • Intermediate computer software knowledge/skill level.
  • Must be able to assess data reflecting the client’s status and interpret the appropriate information needed to identify each client’s requirements relative to their age-specific needs.
  • Must be able to read, write, and communicate in English adequately to complete required reports, interpret Plan of Care instructions, and interact with the client/family, physicians, others.
  • Must have the ability to exhibit professionalism, tact and patience when communicating with patients, clients, family’s subordinate staff, staff at all levels and components of the organization, business associates, and the public.
  • Must have knowledgeable of the regulations associated with prevention of occupational disease and injury, including the exercise of universal precautions and the prevention of contamination.
  • Must have the ability to plan and organize duties, including managing multiple tasks; ability to work under pressure with frequent interruptions; and ability to work independently, with a minimum of supervision.
Experience:
  • Two (2) years work experience as a hospice or end of life care nurse, preferred.
  • Demonstrated experience in coordination of activities and hospice nursing.
  • Demonstrated ability to relate effectively with patients, staff, physicians, community agencies, and institutions.
  • Demonstrated ability to communicate effectively with members of the hospice interdisciplinary team (IDT).
SPECIAL WORKING CONDITIONS
  • The associate will be subject to outdoor elements while accessing a patient’s residence.
  • Will travel daily in the greater Kansas City metro area; must be able to provide own transportation.
  • May be exposed to infectious diseases.
  • Required to function around individuals with challenged mental capacities.

  • All associates may be called upon to assist other departments in a declared emergency situation.
