RN-Transplant Coordinator: Lung Transplant

The Lung Transplant Coordinators report directly to the physician of the Lung Transplant programs with administrative oversight by the Transplant Center Director or their designee. The coordinators are responsible for ensuring patients' continuity of care in all phases of lung transplantation. They serve as the central point of reference for all information about patients participating in the program. The scope of the position includes patient care, education, quality assurance, research, administrative and financial activities, and coordination of transplant care with medical and surgical staff as appropriate.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Patient Care:

  • Serve as the primary point of contact for patients in the pre or post-transplant phase
  • Coordinate all aspects of the transplant process, including patient assessments, and pre or post-transplant care
  • Provides care, within his or her scope of practice and licensure in the outpatient setting to achieve optimal physical, social, and emotional rehabilitation.
  • Identifies and responds to recipient/family/legal guardian's educational, psychosocial, and economic needs in the post-transplant phase.
  • Reinforces education and follow-up requirements necessary for successful outcome.
  • Observes, monitors, and assesses recipient's compliance with medications and medical regimen; collaborates with the transplant team to attain and maintain compliance.
  • Collects and reviews data pertinent to recipient's health and organ function and ensures that the information is documented in the patient's chart.
  • Communicates lab data and diagnostic results to transplant physician/health care provider.
  • Implements changes in drug/treatment therapy as ordered by physicians and other credentialed health care providers.
  • Understands pre and post-transplant protocols and acts as a resource for internal and external health care providers.(i.e., high risk labs/NAT testing 4-8 weeks after transplantation).
  • Arranges for consultations, diagnostic procedures and hospitalization when indicated.
  • Maintains communication with referring physician, and other health care providers.
  • Provides and maintains comprehensive documentation in patient medical record.
  • Participate in multidisciplinary team meetings to discuss patient progress and treatment plans.
Research:
  • Examines current transplant-related research.
  • Participates in and supports research activities within the Transplant Program and MGH Transplant Center and publishes results as appropriate.
Education:
  • Initiate, participate, and contribute to education programs for staff at the unit/service/Center levels.
  • Maintain clinical skills and certification as appropriate to role.
Administrative and Financial:
  • Participate in developing, implementing, and evaluating on an ongoing basis guidelines, protocols, and standards.
  • Maintains system of record-keeping on all potential transplant recipients.
  • Is knowledgeable regarding requirements for documentation, data submission, quality assurance and other reporting of UNOS, NEOB, CMS and other regulatory bodies and ensures compliance with Policy and Procedure for Data Submission to the OPTN.
  • Collaborates among MGH organ-specific transplant programs to ensure efficient and effective use of resources and expertise.
  • Initiate and participate in required quality assurance activities.
  • Participate on committees and task forces that relate to organ transplantation as requested by Physician Director(s) and/or Transplant Center Director (or designee).
  • Maintain data in clinical records and databases as appropriate to evaluation/clinical care and consistent with Transplant Center/organ-specific policies and procedures.
Professional Development:
  • Completes/ensures completion of appropriate documentation in accordance with Institutional/Transplant Program/Center policies and procedures
  • Maintains own clinical competence via ongoing education.
  • Reviews and integrates into practice the current professional literature in the field of transplantation.
  • Attends practice-related conferences.
  • Participates in professional transplant/procurement organizations.
  • Shares knowledge through mentoring/precepting other Transplant Coordinators and transplant staff as needed.
Professional Practice:
  • Protects the rights of patients, their families/legal guardians, and health care team members.
  • Respects individual privacy and holds confidential all information obtained in the course of practice.
  • Assures open and clear communication with patients, families and health care team members without bias or discrimination.
  • Assures that quality and performance improvement standards are developed, implemented, and maintained.
  • Evaluates self-performance by comparing actual outcomes to expectations.
  • Monitors recipient referrals and transplant outcomes to meet performance.
  • Maintains cooperative relationships with other health care professionals.
  • Acts to protect the public trust when health care and safety are endangered.
  • Assists OPOs and other community organizations with educational presentations and information appropriate to the needs of transplant professionals and the public.
  • Reviews, evaluates, and revises education materials and programs for transplant related practice.
Qualifications
  • Current Massachusetts licensure as a registered nurse
  • Bachelor's degree in nursing or equivalent experience required.
  • Master's degree in a clinical nursing specialty preferred.
  • Minimum of 3 years recent Intensive Care Unit and/or transplant experience required
