The post -transplant nurse coordinator reports to the abdominal transplant clinic nurse manager and the administrative and physician directors of the abdominal transplant programs. The post-coordinator is a registered nurse who is responsible for assuring competent, compassionate assessment and care of outpatients presenting to the abdominal transplant clinic. The transplant coordinator works with a multidisciplinary team of health care professionals including physicians, surgeons, pharmacists, social workers, dietitians, financial coordinators, medical assistants, research and administrative staff, to provide comprehensive and individualized care for abdominal transplant recipients. The coordinator serves as the central point of reference for all information about patients. Incumbent is responsible for coordinating the administrative and clinical aspects of the patients, including patient care, education, quality assurance, research, administrative and financial activities, and consultation. The coordinator will screen patients, educate patients/families/staff, assist in the preparation of clinic procedures, coordinating the patient's plan of care, verifying test results and communicating between the patient and the physician/ advanced practice provider. The transplant coordinator also interacts with external stakeholders such as organ procurement organizations, transplant centers, insurance companies, pharmaceutical representatives, and community providers. The transplant coordinator may supervise, or mentor other nurses or students involved in the transplant program.

Duties and Responsibilities

  • Provides care, within scope of practice and licensure in the outpatient setting to maintain optimum quality assurance in all aspects of patient care.
  • Identifies and responds to recipient/family/legal guardian's educational, psychosocial and economic needs in the post-transplant phase.
  • Conduct assessment of patient need
  • Obtains brief history and relevant patient history, physical examination, and reasons for the visit, and records this information. Collects and reviews data pertinent to recipient's health and organ function and ensures that the information is documented in the patient's electronic record. Reviews patient history and other supporting clinical data to facilitate optimum care.
  • Provide direct care to patients. This includes the administration of medications, IV therapy/ infusions (in absence of infusion nurse), injections, lab collection from AV fistula/graft or PICC/ports, POCT testing, wound care and other clinical services per nursing clinical guidelines/policies. Assist physician and/or advanced practice provider with procedures or patient care in the absence of the medical assistant.
  • Effectively explains and educates patients and/or family members about any necessary procedures, surgeries, and post-transplant expectations including potential complications. Reinforces education and follow-up requirements necessary for successful outcome. Teaching is based on assessment, expressed need from patient or family, and/or on the initiation of new treatment. Provide reinforcement of education materials provided in written/electronic version of post-transplant education binder.
  • Provides and maintains comprehensive documentation of recipient progress: documents and records all visits, procedures and encounters. Documentation must follow nursing clinical guidelines/policies for each patient encounter (includes direct contact and telephone encounters) using electronic medical record.
  • Assembles all necessary clinical, laboratory, and other administrative information and data to recommend care plan to and for review by the physician and/or advanced practice provider.
  • Monitor and document patient progress and outcomes after transplantation, including laboratory tests, vital signs, symptoms, complications, and interventions. Collects and reviews data pertinent to recipient's health and organ function and ensures that the information is documented in the patient's electronic record.
  • Communicates the plan of care and other pertinent information to/from other healthcare providers and payors as needed. Communicates directly with members of the multidisciplinary team, using electronic medical record communication as indicated to ensure quick and accurate patient care. Serve as a liaison with other health care providers, i.e. primary care physicians, other specialty physicians, in-patient nursing, visiting nurses, and insurance providers.
  • Performs all follow up to support patient needs to include follow-up of patients having laboratory values not attached to clinic visits and follow-up of patients per physician and/or advanced practice provider orders.
  • Collaborates with in-patient nursing regarding patient admission, discharges, and/or patients requiring follow-up after clinic hours.
  • Communicates lab data and diagnostic results to transplant physician/advanced practice provider and/or other health care provider.
  • Implements changes in drug/treatment therapy as ordered by physicians/advanced practice provider and other health care providers. Assures informed consent for procedures and transplant processes as appropriate. Assures access for patients to treatment alternatives.
  • Assures open and clear communication with patients, families and health care team members without bias or discrimination. Protects the rights of patients, their families/legal guardians, and health care team members. Respects individual privacy and holds confidential all information obtained in the course of practice.
  • Manages medication refills and prior authorizations to ensure patients maintain appropriate medication management.
  • Observes, monitors, and assesses recipient's compliance with medications and medical regimen, lab protocols for monitoring patient and graft outcomes; collaborates with the transplant team to attain and maintain compliance.
  • Collaborate with the Patient Services Coordinators in scheduling appointments including follow-up, infusion, and laboratory appointments. Arranges for consultations, diagnostic procedures and hospitalization when indicated or in the absence of the Patient Services Coordinator. Collaborate with the Patient Services Coordinators to follow-up with patients missing clinic/laboratory appointments and/or lost to follow-up.
  • Manage and maintain active patient data reported to the New England Organ Bank and UNOS.
  • Reviews, evaluates, and revises education materials and programs for transplant related practice.
  • Understands post-transplant protocols and acts as a resource for internal and external health care providers.
  • Works with physician and office staff to maintain equipment. Participates in routine care and maintenance of equipment.
  • Participate in quality improvement activities, research projects, policy development and professional education related to transplantation. Possess a global awareness of the daily schedule both for the individual as well as the entire staff.
  • Performs new techniques and/or routines as necessitated by technological advances/changes. Maintains and updates clinical knowledge and skills based on current nursing education practices.
Qualifications
  • Current licensure as a Registered Nurse in the Commonwealth of Massachusetts required.
  • Current BLS certification required.
  • Bachelor's degree in nursing or equivalent experience required.
  • Master's degree in a clinical nursing specialty preferred.
  • Transplant Certification (CCTC) preferred.
  • 1 - 2 years' inpatient nursing experience required. Critical care or transplant nursing experience preferred.
