Reason position exists: The Case Manager’s primary role is to provide intensive on-going case management services and supervision of an assigned caseload of adult clients within a residential, sober, supportive housing site over the length of their stay by performing the following duties. This is located at Clifton Place in Minneapolis and is an Adult Residential Half Way House.

Duties and Responsibilities: include the following. Other duties may be assigned.

  1. Meet with potential clients inside correctional facilities for orientation as needed.
  1. Complete intake process and make appropriate referrals within 24 to 48 hours.
  1. Assist clients with eligibility for GRH, medical and county benefits.
  1. Knowledgeable about the housing market for clients with criminal backgrounds.
  1. Coordinate with clients to meet their employment, housing, mental health, CD and cognitive needs with other professionals in the community.
  1. Communicate with client’s professional contacts including but not limited to Correctional professionals, Mental Health, Chemical Health, and Medical initially and on-going.
  1. Create comprehensive case plans at entry to the program, update every 30-days and upon exit.
  1. Keeps updated Individual Case Plans, Case notes, and any other programmatic forms as directed by the Senior Manager.
  1. Submit bi-weekly progress reports to correctional professionals.
  1. Complete termination and discharge reports to be submitted to correctional professionals within 48 hours.
  1. Manage and provide direct services to clients to monitor compliance with court ordered conditions of release and remain law abiding.
  1. Facilitate staffing with clients in a timely matter who experience violations and face termination with community collaterals and PO’s.
  1. Have at least one face to face contact with each client assigned to caseload a minimum 1x week.
  1. Create with clients, sign and monitor client’s daily planners for accountability purposes.
  1. Monitor client’s progress in maintaining health and wellness, income, housing, employment opportunities, and individualized topics such as chemical dependency programming, psychological/mental health resources, and accessing appropriate benefits opportunities.
  1. Chrono clients contact and service coordination into Power 180 computer system in an accurate and timely manner.
  1. Transport or arrange transportation for clients to medical, court, or other appointments.
  1. Attend community meetings relevant to program and clients.
  1. Maintain knowledge of the Department of Correction system, relevant laws, regulations and policies regarding reporting requirements, data practices, and client rights.
  1. Monitor/maintain public and facility safety within job assignments, includes monitoring safety and security of 180 if assigned.
  1. Maintain flexibility to work in a residential 24 hours facility including evenings, holidays, weekends and overnights as needed to meet the needs of clients and the agency.

Minimum Qualifications:

Associate degree in Corrections, Criminal Justice, Sociology, Psychology, Social Work, Education or related behavioral science field AND

One-year paraprofessional experience in corrections or human service agency.

OR an equivalent combination of related education and experience.

PREFERRED EDUCATION BEYOND MINIMUM QUALIFICATIONS:

Baccalaureate degree in Corrections, Criminal Justice, Sociology, Psychology, Social Work, Education or related behavioral science field.

Special Skills:

Culturally competent

Knowledge of community resources and other programs available to clients such as chemical dependency programs, mental health treatment, education programs, individual counseling, cognitive skills groups and culturally specific services available in the community.

Performs effective time management

Ability to communicate effectively, both verbally and written

Ability to establish and maintain positive working relationships with clients, coworkers, law enforcement officials, judges and treatment facilities

Ability to work under stress and use discretion in making decisions

Ability to advise people in crisis situations and make decision leading to crisis resolution

Ability to prioritize work according to clients and agency need

Ability to assess a person's social, economic, emotional, mental, and health needs

Computer Skills:

To perform this job successfully, an individual should have knowledge of Microsoft Office Suite and basic computer skills.

Certificates and Licenses:

Valid driver’s license

Valid driver license

CPR/First Aid certificate within 6 months of hire

Medication Certificate within 6 months of hire
