Case Manager – PATH Grant

Starting Pay: $65,000 annual

Location: Minneapolis (Seward neighborhood)

Schedule: Five weekdays (8-hours each) or Four weekdays (10-hours each)

The Case Manager will provide outreach and housing assistance for individuals experiencing homelessness in Hennepin County, as part of the PATH (Projects for Assistance in Transition from Homelessness) grant.

Case Manager – PATH Grant:
  • Connect with individuals referred to Touchstone via Hennepin County’s Coordinated Entry Program.
  • Locate individuals in the community and provide information on Touchstone’s programs.
  • Support individuals in accessing social security benefits and community resources.
  • Support individuals in completing the admissions processes for Touchstone’s programs.
  • Support individuals as they transition into housing as part of Touchstone’s programs.
  • Collaborate closely with other staff members in connecting and locating individuals.
  • Document client care in the electronic health record (Credible).
  • Maintain records to demonstrate outcomes as required by PATH grant.
Case Manager – PATH Grant:
  • Must maintain a valid driver’s license and acceptable driving record.
  • Must maintain acceptable vehicle insurance.
  • Experience supporting individuals experiencing homeless strongly preferred.
  • Experience using evidence-based practices such as housing first, motivational interviewing, and harm reduction strongly preferred.
  • Experience providing person-centered, trauma-informed support strongly preferred.
  • Experience supporting adults with mental illness, substance use, and/or co-occurring disorders preferred.
  • Experience working within the community preferred.
