Family Community Coordinator

Responsible for following up on referrals and providing information about the program and services. Responsible for scheduling and assisting with patient discharges with a particular focus on discharge planning. Responsible for participating in treatment planning for child and adolescent population as assigned. Responsible for maintaining and updating a transportation manifest. Responsible for case management of child and adolescent PHP patients and being a liaison with schools, DJJ officers, medical doctors, family, DFACS, and all other possible referral sources or services.
