The Impact You Can Make The Psychiatric Case Manager is the primary team member responsible for formulation of a patient’s treatment and discharge plan. Under the general direction of the Nurse Manager of the Inpatient Behavioral Health Unit, the Psychiatric Case Manager coordinates and collaborates with all members of the treatment team, including the patient, psychiatrist, nurse, program coordinator, etc., to establish a robust and comprehensive treatment and discharge plan. The Psychiatric Case Manager uses case management and clinical knowledge to promote appropriate allocation of hospital and other resources, while striving to provide high quality care to each patient. The Psychiatric Case Manager coordinates with the patient’s support network, including family, outpatient providers and therapists, community service organizations, other facilities, etc., to ensure that the patients psychiatric and social needs are fully understood and met. ** The Glens Falls Hospital Impact Mission * * Our Mission is to improve the health of people in our region by providing access to exceptional, affordable, and patient-centered care every day and in every setting. * * * * How You Will Fulfill Your Potential Responsibilities * Assess the patient’s plan of care as soon as possible upon admission and re-evaluates at least every week or more often as indicated. * Is the primary team member responsible for patient discharge planning * Identifies barriers to discharge in a timely manner so that they can be addressed without resulting in a delay of discharge. * Ensures completion of all appropriate discharge forms within the established timeframes. * Completes a social history on each patient and documents appropriately * Participates in multi-disciplinary treatment team meetings daily * Serves as a liaison on behalf of the patient between the unit and outside agencies * Conducts family meetings to assess, evaluate, and identify discharge needs. * Ensures patient’s mental hygiene legal status documentation is completed appropriately * Coordinates with outside agencies and entities to ensure that post-discharge plans are appropriate and timely. Education/Accredited Programs * Bachelor’s Degree required, Master’s Degree preferred * Two years acute care case management preferred * Experience in psychiatric case management preferred * Experience in inpatient psychiatric setting preferred ** Licenses/Certifications/Registrations * Maintains current licensure and/or certification in appropriate professional affiliation ** Skills/Abilities * Ability to function autonomously maintaining a high level of clinical and professional accountability. * Demonstrates skill in creative problem solving, facilitation, collaboration, coordination, and critical thinking. * Embraces change and continuously identifies opportunities for improvement by demonstrating a commitment to creativity and innovation. * Committed to promoting excellence in customer service.
