Job Title: Case Manager

At Klamath Basin Behavioral Health, we are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing services that directly improve the lives of individuals diagnosed with Serious and Persistent Mental Illness (SPMI).

Key Responsibilities:
  • Partner with clients to develop person-centered recovery plans, focusing on their strengths and goals for self-sufficiency.
  • Conduct housing searches, provide rental assistance, and offer eviction prevention support to ensure clients' housing stability.
  • Deliver outreach and life-skills training across multiple domains, including employment, finance, health, legal, and social integration.
  • Maintain consistent documentation and regular contact with clients, including home visits, office visits, and phone communications.
  • Collaborate with landlords to resolve housing challenges and provide ongoing support to clients.
  • Participate in clinical meetings, training, and supervision sessions, applying new skills and knowledge to case management.
Requirements:
  • Bachelor's degree in a behavioral science field preferred, or a combination of three years of relevant work, education, or training.
  • Valid Oregon driver's license and personal auto insurance.
Preferred Qualifications:
  • Experience working with adults diagnosed with mental illness or disabilities.
