Job Title: Registered Nurse (RN) - Case Manager (Acute Care)

About the Role:

The Care Manager (Acute Care) role at Glens Falls Hospital is a critical position that requires a skilled and compassionate Registered Nurse to optimize and integrate the interface between clinical and financial systems, supporting positive health outcomes. This role is responsible for assessing, integrating, and facilitating patient discharge plans, focusing on the coordination of care for medium to high-risk patients across the healthcare continuum.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans to ensure a "reasonably safe discharge plan"
  • Collaborate with healthcare teams to prevent fragmentation and duplication of services
  • Work with physicians and other healthcare professionals to meet patient-specific and age-related needs
  • Ensure delivery of cost-effective, outcome-oriented, and quality care within an expected length of stay

Requirements:

  • RN or Social Work degree preferred (MSW or BSW)
  • Human services degree with 2 years of direct care experience accepted
  • Home care, hospital, or nursing home experience strongly preferred

Benefits:

Glens Falls Hospital offers a competitive salary range of $32.00 to $58.74 per hour, commensurate with experience level, education, and certifications. Additionally, we provide valuable and competitive benefits offerings to our employees.
