Job Title: Case Manager

Join our team at Columbiana County Mental Health Clinic, dba The Counseling Center, as a Case Manager. In this role, you will provide direct services to clients, families, and specific target populations, per assigned agency or off-site locations.

Key Responsibilities:
  • Provide ongoing services and crisis management to clients with dual diagnosis of substance abuse and mental illness, and/or per assigned population.
  • Manage caseloads, including court-ordered individuals and specific DJFS referrals.
  • Develop and implement community services that address clients' principal needs.
  • Provide group therapy as assigned and record documentation, service planning, and progress of clients in achieving treatment goals.
  • Participate in individual supervision, group supervision, peer review activities, team meetings, and quality assurance meetings.
  • Travel, case consultation, telephone contacts, education, and prevention services which are non-billable.
  • Meet productivity expectations and timely and accurate completion of documentation.
Requirements:
  • Bachelor's degree in a field of mental health (social work, counseling, psychology, human/family services) required.
  • Ohio chemical dependency (CD) certification/licensure in substance abuse preferred.
  • Minimum of two years' related experience and same Ohio CD certification/licensure required.
Preferred Qualifications:
  • Bachelor's degree in a related field of mental health, such as social work, counseling, psychology, human/family services.
  • One year paid experience, preferably with service population, and Ohio chemical dependency certification/licensure.
